Why Your Shoulders Actually Hurt on Bench Press

Shoulder pain on the bench press is one of the most frustrating problems in the gym. You're not doing anything that feels wrong. You're pressing, like you always have. But somewhere in that movement, at the bottom of the rep, during the lockout, or just as the fatigue sets in, something in your shoulder lights up.

The instinct is to blame the shoulder itself. Ice it, rest it, maybe switch to dumbbells for a while. And sometimes that's enough to get you through the next few weeks.

But if the pain keeps coming back, the shoulder is probably not where the problem actually starts.

The bench press is a full upper-body movement that demands coordinated input from three separate systems: your shoulder joint, your scapula, and your thoracic spine. When any one of those three isn't doing its job, the shoulder pays for it.

The Three Root Causes Most People Miss

1. Shoulder Rotation: The Joint Isn't Ready for the Position

The shoulder is a ball-and-socket joint with enormous potential range of motion. That's what makes it so versatile; and so vulnerable. During the bench press, the humerus (upper arm bone) needs to move through a combination of horizontal adduction and internal rotation as you press, then reverse through external rotation as you lower the bar back to your chest.

When internal or external rotation is restricted, which is extremely common in people who train pressing movements heavily without balancing them, the shoulder doesn't have the range to move cleanly through the rep. It improvises. The joint translates forward, the rotator cuff gets compressed, or the anterior capsule gets loaded in ways it shouldn't.

That impingement-like pain at the front of your shoulder? In most cases, it's a rotation deficit, not structural damage.

2. Scapular Control: The Platform Isn't Stable

The scapula is the foundation the shoulder joint sits on. Every pressing movement depends on the scapula being able to move correctly, retracting and depressing to create a stable base when the bar is at your chest, then protracting (moving forward around the ribcage) as you press to the top.

Most people are told to "pull back their shoulder blades" and keep them pinned back throughout the entire lift. That cue has some merit for powerlifting, but for general training it creates a problem: if the scapula can't protract at the top of the press, the shoulder joint has to pick up that range of motion on its own. That means more anterior translation of the humeral head, more impingement risk, and more load on tissue that wasn't designed to absorb it.

The other side of the issue is equally common, scapulae that can't retract and stabilize at all, leaving the shoulder joint pressing off an unstable, winging base. Both failures create shoulder pain. They just create it in slightly different spots.

3. Thoracic Spine Rotation: The Engine Room Nobody Checks

This is the one almost nobody talks about in the context of bench press shoulder pain, but it may be the most important.

The thoracic spine, your mid and upper back, is designed to rotate, extend, and flex. When it's stiff (which it is in nearly everyone who sits for long periods or trains without addressing it), it doesn't contribute the spinal rotation and extension that a healthy press requires. That stiffness transfers directly into the shoulder complex above it.

Think of it this way: the thoracic spine, scapula, and shoulder joint are a chain. A full pressing motion requires coordinated movement across all three. If the thoracic spine is locked down and can't rotate or extend, the scapula can't move efficiently around the ribcage, and the shoulder joint ends up compensating with excessive range it wasn't built to provide alone.

Bench press shoulder pain that doesn't improve with shoulder-specific work is almost always a thoracic spine problem in disguise.

What to Do About It

Assess your shoulder rotation. Test your active internal and external rotation; not just passive flexibility, but whether you can actively control those ranges. If your external rotation is limited, the rotator cuff and posterior capsule need work. If internal rotation is the issue, you likely have anterior tightness and poor posterior shoulder control. Both are addressable, but they require different interventions.

Train scapular movement, not just position. The scapula needs to be able to retract, protract, elevate, and depress with control; and transition fluidly between those positions under load. Exercises that isolate scapular movement patterns (serratus anterior activation, scapular CARs, wall slides) build the kind of dynamic stability the bench press actually demands. "Squeeze and hold" isn't enough.

Restore thoracic rotation and extension. Thoracic spine CARs, segmental extension work, and rotation drills directly upstream of your pressing sessions can make an immediate and significant difference. A thoracic spine that moves freely changes how your scapula sits, which changes how your shoulder loads, which changes whether or not bench press hurts.

Relearn the press pattern. Once your upstream mobility is improved, it's worth rebuilding your pressing mechanics from scratch, lighter weight, deliberate tempo, focus on the shoulder's path through the rep rather than just moving weight. Most pain-producing pressing habits formed gradually and need to be actively unlearned.

Balance your pressing with pulling. Every horizontal press should be matched with horizontal pulling work. This isn't just a cue for symmetry; it maintains the rotational balance of the shoulder joint and keeps the posterior shoulder and scapular retractors strong enough to do their job when it counts.

The Bigger Picture

Shoulder pain on the bench press isn't a bench press problem. It's a system problem, a breakdown somewhere in the chain of thoracic spine, scapula, and shoulder joint that forces one structure to absorb load it shouldn't be handling alone.

You don't fix a chain problem by replacing one link. You fix it by understanding how the whole thing is supposed to move, identifying where it's failing, and building the capacity back systematically.
